Consultation Consent

I consent for Balbir Daly to work with my infants(s) and myself during this and subsequent consultations regarding breastfeeding problems/ concerns. I understand that this may involve observation of me breastfeeding and/ or examination of my breasts, performing an oral digital examination of my infant(s) to access sucking, and demonstrate and/or use of equipment and techniques as may assist in improving breastfeeding and ensuring adequate nutrition for my infant(s).

I consent for Balbir Daly to share pertinent information about our consultations with my GP, PHN or other health care provider.

I understand that all of my medical care is provided by my GP or other medical doctor.

I understand that I am solely responsible for payment for all services and supplies on the day.

I understand that I have the right to refuse any or all specific techniques suggested, equipment offered. or recommended actions.

I understand it is my responsibility to call the lactation consultant if necessary, with progress reports, questions or concerns.

I give consent for the lactation consultant to use clinical information obtained during our sessions for education of other health care providers and mothers about lactation.I will not be identified in any way but aspects of my situation may be discussed and described.

I waive, release and forever discharge all claims against Balbir Daly for any injuries, damages, losses, claims or losses, whether known or unknown, which arise during or as a result from any activity or services provided.
